Werkzaamheid van onderhoudstherapie bij MSS-CRC: Fluoropyrimidine + Bevacizumab
? Bevacizumab plus een fluoropyrimidine is de eerste lijn van onderhoudstherapie die GRANITE probeert te verslaan.
Er zijn een paar baanbrekende onderzoeken die als basis hebben gediend voor deze eerstelijns onderhoudstherapie.
CAIRO3 – capecitabine plus bevacizumab kwam aan bod in de Nederlandse CAIRO3-studie, waarin 558 patiënten met een stabiele ziekte of beter na zes cycli XELOX plus bevacizumab, die niet in aanmerking kwamen voor potentieel curatieve metastasectomie, willekeurig werden toegewezen om door te gaan met capecitabine (625 mg/m2 tweemaal daags elke dag). ) plus bevacizumab (7,5 mg/kg elke drie weken) of alleen observatie. Bij de eerste progressie (PFS1) moesten patiënten in beide armen volgens protocol worden behandeld met XELOX plus bevacizumab tot de tweede progressie (PFS2). Het primaire eindpunt was PFS2, berekend vanaf het moment van randomisatie. Onderhoudstherapie ging gepaard met een significant langere PFS2 (11,7 versus 8,5 maanden, risicoratio [HR] 0,67, p<0,0001) , en er was ook een trend richting verbeterde algehele overleving ( mediaan 21,6 versus 18,1 maanden, HR 0,89, p = 0,22 ).
• Duitse AIO KRK 0207-studie – Op vergelijkbare wijze werd een voordeel voor voortgezette behandeling met fluoropyrimidine plus bevacizumab vergeleken met alleen observatie ook aangetoond in de Duitse AIO KRK 0207-studie, waarin patiënten zonder progressieve ziekte na zes maanden oxaliplatine plus een fluoropyrimidine en bevacizumab willekeurig werden behandeld. toegewezen aan onderhoudsbehandeling met hetzelfde fluoropyrimidine plus bevacizumab, alleen bevacizumab of alleen observatie. Het primaire eindpunt was de ‘tijd tot falen van de strategie’ of TFS, die de duur van het onderhoud omvatte plus de tijd vanaf herinductie na de eerste progressie tot een tweede ziekteprogressie. De proef was bedoeld om non-inferioriteit aan te tonen met een non-inferioriteitsmarge van 3,5 maanden, wat overeenkomt met een HR van 1,42. De mediane TFS in de fluoropyrimidine plus bevacizumab- en observatiearmen was niet significant verschillend (respectievelijk 6,9 en 6,4 maanden; HR 1,26, 95% BI 0,99-1,60). De observatie-arm was echter niet non-inferieur aan fluoropyrimidine plus bevacizumab, omdat de bovengrens van het 95 procent betrouwbaarheidsinterval de drempel voor non-inferioriteit overschreed (1,43). Opvallend is dat weinig patiënten in beide armen werden blootgesteld aan een herinductiebehandeling (19 procent met gecombineerde therapie en 46 procent van degenen die observatie ondergingen), waardoor het primaire eindpunt, TFS, niet-informatief en klinisch irrelevant werd.
• STOP en GO-studie – Een enigszins andere aanpak werd getest in de Turkse STOP en GO-studie, waarin, na zes cycli bevacizumab plus XELOX, 123 patiënten willekeurig werden toegewezen aan voortzetting van de behandeling of stopzetting van oxaliplatine en onderhoud met bevacizumab plus capecitabine totdat progressie. De mediane PFS was significant beter in de groep die onderhoudstherapie kreeg met bevacizumab plus capecitabine ( 11 versus 8,3 maanden ), met minder graad 3 of 4 diarree (3,3 versus 11,3 procent), hand-voetsyndroom (1,6 versus 3,2 procent) en neuropathie. (1,6 versus 8,1 procent).

Efficacy of Maintenance Therapy in MSS-CRC: Fluoropyrimidine + Bevacizumab
?Bevacizumab plus a fluoropyrimidine is the first line of maintenance therapy that GRANITE tries to beat.
There're a couple landmark trials that used as the basis for this 1st line maintenance therapy.
CAIRO3 – capecitabine plus bevacizumab was addressed in the Dutch CAIRO3 trial, which randomly assigned 558 patients with stable disease or better after six cycles of XELOX plus bevacizumab who were not eligible for potentially curative metastasectomy to continued capecitabine (625 mg/m2 twice daily every day) plus bevacizumab (7.5 mg/kg every three weeks) or observation alone. Upon first progression (PFS1), patients in both arms were supposed to be treated with XELOX plus bevacizumab until the second progression (PFS2) per protocol. The primary endpoint was PFS2, which was calculated from the time of randomization. Maintenance therapy was associated with a significantly longer PFS2 (11.7 versus 8.5 months, hazard ratio [HR] 0.67, p<0.0001), and there was a trend toward improved overall survival, as well (median 21.6 versus 18.1 months, HR 0.89, p = 0.22).
•German AIO KRK 0207 trial – Similarly, a benefit for continued fluoropyrimidine plus bevacizumab as compared with observation alone was also shown in the German AIO KRK 0207 trial, in which patients without progressive disease after six months of oxaliplatin plus a fluoropyrimidine and bevacizumab were randomly assigned to maintenance with the same fluoropyrimidine plus bevacizumab, bevacizumab alone, or observation only. The primary endpoint was the "time to failure of strategy" or TFS, which included the duration of maintenance plus the time from reinduction after first progression to a second disease progression. The trial was powered to demonstrate noninferiority with a noninferiority margin set at 3.5 months, corresponding to an HR of 1.42. The median TFS in the fluoropyrimidine plus bevacizumab and observations arms was not significantly different (6.9 and 6.4 months, respectively; HR 1.26, 95% CI 0.99-1.60). However, the observation arm was not non-inferior to fluoropyrimidine plus bevacizumab because the upper limit of the 95 percent confidence interval exceeded the threshold set for non-inferiority (1.43). Notably, few patients in either arm were exposed to reinduction treatment (19 percent with combined therapy, and 46 percent of those undergoing observation), rendering the primary endpoint, TFS, non-informative and clinically irrelevant.
•STOP and GO trial – A slightly different approach was tested in the Turkish STOP and GO trial, in which, following six cycles of bevacizumab plus XELOX, 123 patients were randomly assigned to continued therapy or discontinuation of oxaliplatin and maintenance with bevacizumab plus capecitabine until progression. The median PFS was significantly better in the group receiving maintenance therapy with bevacizumab plus capecitabine (11 versus 8.3 months), with less grade 3 or 4 diarrhea (3.3 versus 11.3 percent), hand-foot syndrome (1.6 versus 3.2 percent), and neuropathy (1.6 versus 8.1 percent).
